LOS ANGELES, Dec 3 (Reuters) - Ketamine, the powerful sedative that caused actor Matthew Perry's 2023 drug overdose death, has been approved for clinical use as an anesthetic since 1970, when it was first given to wounded American soldiers during surgery in the Vietnam War.

More recently, the drug has found renewed favor ​in the medical establishment for various applications ranging from pain management to treatment of some psychiatric disorders including severe depression. Ketamine also has gained growing popularity as an ‌illicit party drug.

Five people, including two physicians, who were accused of illegally procuring ketamine for Perry pleaded guilty to federal drug charges brought following the actor's death in Los Angeles. Dr. Salvador Plasencia is due in federal court on ‌Wednesday as the first of these defendants to be sentenced.

Here is an explanation of what ketamine is and how it has been used.

WHAT IS KETAMINE AND HOW IS IT GIVEN?

Labeled as a "dissociative anesthetic hallucinogen" because it produces a feeling of detachment from pain, anxiety and the environment, ketamine can be injected, mixed in solution for infusion or absorbed under the tongue in dissolving tablet form.

In January 2025, the U.S. Food and Drug Administration expanded approval for Johnson & Johnson's ketamine-derived nasal spray Spravato as a stand-alone medication for patients with severe depression resistant to other forms of treatment.

It was previously approved for ⁠use in combination with antidepressants, and later for patients experiencing suicidal ‌thoughts or actions.

The fast-acting drug works by targeting the neurotransmitter glutamate.

According to the U.S. Drug Enforcement Administration, ketamine is often snorted as a powder or smoked by individuals abusing it for recreational purposes.

HOW IS KETAMINE USED AS A STREET DRUG?

Ketamine has become increasingly popular among recreational drug users, a trend associated ‍with the proliferation of nightclub "rave" culture. Classified by the DEA as a Schedule III controlled substance - defined as having a moderate to low potential for abuse - ketamine has assumed various street names over the years including "Vitamin K," "Kit Kat" and "special K."

In the case of Perry, who starred on the hit TV sitcom "Friends" and publicly acknowledged a history of substance abuse, the road to his fatal overdose began with ketamine infusions he received ​at a licensed clinic for treatment of depression. It ended in his hot tub after Perry resorted to unsupervised at-home injections of the drug, relying on an illegal network of ‌suppliers and enablers to satisfy what became his latest addiction, according to law enforcement officials.

"When clinic doctors refused to increase his dosage, he turned to unscrupulous doctors who saw Perry as a way to make quick money," Anne Milgram, the DEA's director at the time, said in August 2023 when the defendants were charged.

KETAMINE APPLICATIONS BESIDES DEPRESSION

Except in cases involving trauma or severe burn injuries, ketamine is rarely used nowadays as a primary general anesthetic for surgery. This is because in high doses, it can cause hallucinations, raise blood pressure and lead to other unwanted side effects, according to the American Society of Anesthesiologists.

Ketamine is more frequently used for simpler procedures requiring short-term sedation or as a supplemental anesthetic given during surgery to help manage residual pain expected after ⁠the operation, the group said.

A growing body of medical science supports ketamine's benefits in treating a variety of ​neurological conditions including epilepsy, stroke and migraines.

HOW KETAMINE KILLED MATTHEW PERRY

Perry, 54, was found by his live-in assistant floating ​face down and lifeless in the hot tub of his Los Angeles home in October 2023. An autopsy concluded Perry died from the acute effects of ketamine and other factors that caused him to lose consciousness and drown.

Toxicology tests found ketamine in Perry's body at high levels, within a range typically ‍associated with general anesthesia used in monitored surgical ⁠care.

Those concentrations would have overstimulated Perry's heart rate while depressing his breathing, likely leading him to lapse into unconsciousness before his face slipped below the water surface, according to the autopsy report.

Plasencia had shown Perry's live-in personal assistant, Kenneth Iwamasa, how to administer the ketamine. Iwamasa admitted to repeatedly injecting Perry multiple times at the actor's request, including ⁠on the day he died, according to court documents.

EXTRA RISK FACTORS

Dr. David Mahjoubi, an anesthesiologist who is the owner and medical director of the Ketamine Healing Clinic of Los Angeles, said therapy involving the drug is not recommended for ‌people showing symptoms of schizophrenia or bipolar disorder.

Dr. Gerard Sanacora, a Yale University professor of psychiatry, said ketamine therapy can be especially risky for patients with ‌heart disease or high blood pressure.

The 38-year-old TV host was leading the broadcast of the game in Southampton on Tuesday, Dec. 2, when she collapsed during the first few minutes of the show in a scary, on-screen moment.

Woods was caught by her co-hosts Ian Wright and Anita Asante as the show switched to game coverage before going into a five-minute commercial break,The Timesreported.

Presenter Katie Shanahan replaced Woods after the commercial break while announcing that she'd fallen ill.

"As you have probably noticed, we don't have the wonderful Laura Woods because she's just been taken ill, but she's in very good hands, so I'm stepping in at late notice," said Shanahan, perThe Times.

Woods later posted anupdateon her Instagram Stories, sharing that she collapsed due to a "virus" and was "ok," after being medically checked out.

"Gosh, that was a bit weird," she wrote. "Sorry to worry everyone, I'm ok, the wonderful paramedics at [St. Mary's] has said it's probably a virus, just need a bit of rest & hydration."

"I'm really embarrassed that happened on TV, but a big thank you to my colleagues at ITV who have really looked after me tonight. And to Wrighty and Neets for catching me and sorry again," added Woods, referring to her co-hosts.

Mom, stepdad charged in death of Michigan pregnant woman Rebecca Park

Authorities say two family members did their research and created a plan before they brought a pregnant woman into their home, put her into a car and then took her to an area in the northern Michigan woods —  where they forced her on the ground and stabbed her as they cut her baby out of her womb, ultimately causing both of their deaths.

Then, the pair left 22-year-old Rebecca Park there, Wexford County Prosecutor Johanna Carey said during an arraignment on Tuesday, Dec. 2, in Cadillac of Cortney Bartholomew, 40, and Bradly Bartholomew, 47.

The pair,describedby the Cadillac News as Park's biological mother and stepfather, were charged with the following: one count of first-degree premeditated murder and one count of felony murder in connection with Park's death, torture in a place of confinement, conspiracy to commit torture, assault of a pregnant woman with the intention to cause miscarriage or stillbirth, conspiracy to commit that assault and unlawful imprisonment. Both were additionally charged with moving a dead body.

"This is, frankly, evil personified," Carey said.

Carey said that Park's baby died. No other details were immediately available after the arraignment. The Wexford County Prosecutor's Office declined to comment after the court hearing.

The office is handling the prosecution with the help of the Michigan Attorney General Dana Nessel's office, according to a press release.

An attorney representing Cortney Bartholomew said she was disabled and should have a lower bond and home confinement. Judge Corey Wiggins denied bond for both.

Park was 38 weeks pregnant when she was reported missing Nov. 4. Her body was found in the Huron Manistee National Forest on Nov. 25, but her baby was "not present," authorities had said.

"This case involves a truly horrific homicide in which a young woman and her unborn child endured unimaginable suffering at the hands of the Defendants," Carey in a press release.

"The brutality and disregard for human life displayed here are deeply troubling. While these remain allegations until proven in court, the evidence reflects an extraordinary level of callousness and violence. We look forward to presenting the full facts in court."

It's a case that has bewildered the small-town community in Wexford County, considering police had said they were still looking for the baby as of late last month, even after finding Park's body. Twoother family membershad previously been charged after the tragic family affair: her sister, Kimberly Park, and Richard Falor, who wasreportedlyRebecca Park's fiancé.

Detectives say Kimberly Park lied to authorities, telling them that Cortney Bartholomew had Bradly Bartholomew strike Rebecca Park "in the head causing a seizure and then took her body away,"accordingto the felony complaint filed in Cadillac's district court. She was charged with tampering with evidence in a criminal case, lying to a police officer in violation of a crime investigation and false report of a felony.

Falor was charged with drug crimes and pleaded not guilty in court on Dec. 2.

Dozens of community members, who call themselves "Rebecca's voice,"packed the courtroomTuesday to witness the Bartholomews' arraignments, their cars donning messages like "Say Their Names," "Justice for Becca and Baby Park" and "We are her voice."

It was close quarters on the small courtroom benches, but as one community member said aloud: "We're all trauma bonded by it anyways."

When Wiggins first read the charges of assault of a pregnant woman with the intent to cause miscarriage or stillbirth, the crowd gasped. Cries followed, especially after the prosecutor described what the Bartholomews are accused of.

Even before Park's body was found, most, if not all, of those in the courtroom have been boots-on-the-ground helpers.

They protested in front of the last place Park was seen — allegedly the Bartholomews' home —  for nearly 12 hours on Nov. 23. They scoured through the woods trying to find her. And now that charges were announced in the killing, they're demanding justice.

When they left the courtroom, "Rebecca's voice" finally let out sobs. Stacey Davis, Bradly Bartholomew's estranged sister, could barely walk. She told the Free Press that Cortney Bartholomew and her brother wanted a baby but couldn't conceive one. Davis also said:

"I'm here for Rebecca and her baby. I'm not here for my brother."
